RN/LPN/CNA's: currently licensed RNs/ LPN/CNA's are needed for on-call field operations work and case management opportunities across the agency. Prior home health care experience is a plus; excellent patient care skills and proven problem solving abilities necessary. A strong commitment to the Michigan Home Healthcare's mission a must with ability to organize work and complete required documentation in a dependable and thorough manner is essential.
Nurse Assistant: prefer Certified Nursing Assistants (CNA) but will work with those seeking to gain experience and demonstrate an interest in working in a home health care environment. Must have excellent patient care, communication skills, are dependable, and are committed to Michigan Home Healthcare's mission and philosophy of care.
Occupational Therapists: as an integral part of Michigan Home Healthcare's rehabilitation team, this person would have a responsibility for evaluating and treating patients requiring services associated with occupational therapy. This person would be expected to communicate with the physician regarding the patient's treatment plan and progress, and deliver services consistent with policies and procedures governed by Michigan Home Healthcare's rehabilitation department, State of Michigan licensure and code of ethics standards. Part-time positions available.
Speech Therapists: as an integral part of the rehabilitation team this person will be responsible for evaluating and treating patients requiring services associated with speech therapy. Person will be expected to communicate with the physician regarding the patient's treatment plan and progress, deliver services consistent with policies and procedures governing the delivery of patient care by Michigan Home Healthcare's Rehabilitation Department. Person must have current State of Michigan licensure and adhere to the code of ethics and standards. Part-time positions available.
Medical Social Workers: as a qualified member of our health care team, this person will be responsible for: (1) assessment of psychosocial and emotional factors related to the illness and care needs of patients in their homes; (2) recording clinical and progress notes; and (3) coordinating other patient care activities/services to provide the highest quality of patient care in the Wayne, Monroe, and Washtenaw County areas.
Applicant must have:
1. Master Degree from a school of social work accredited by the Council on Social Work Education.
2. Licensed to practice as a social worker in the State of Michigan.
3. A minimum of one year of social work experience in a health care setting required; previous home care experience preferred.
4. Must possess strong patient assessment, patient management and documentation skills.
5. Must possess the disposition and personality to work harmoniously with a wide variety of people and all levels of staff, and interact professionally as a member of a multi-disciplinary team.
6. Must possess a strong commitment to the goals, mission, and philosophy of the organization.
7. Ability to work flexible hours as required to meet identified patient needs.
